Self Rating Survey for Multi Sport

In speaking with Mike, he wanted to add a quick survey to the self-rating field in all of the sports that we now offer, other than Pickleball. Being there is no existing rating standard, most people will not know where they fall. So when we ask them to sell for a sport like Pickleball, the goal is to create a simple survey with a set of questions that will help them come up with a self rating.
